DOG SURRENDER FORM
If possible, please email clear pictures of the individual dog(s) i.e. no other dogs, cats, kids, people, etc... in the picture to safesnout@gmail.com. Your dog should be delivered to the Doxie & Friends Rescue Team collared, leashed and reasonably groomed (bathed, nails clipped, ears clean).If you are unable to do this we can help . Please bring any favourite toys, blankets, training treats and an appropriately sized crate or kennel. Also include the dog’s current food, along with any information regarding specific food allergies, if applicable. Lastly, include all veterinary records and a month's supply of medications, if needed. The surrender fee is by donation, unless other arrangements have been made. If you need help transporting your dog to the Doxie & Friends Rescue Team please feel free to ask; we have transportation volunteers who can assist. A representative of the Doxie & Friends Rescue Team will be in touch with you as soon as your information is reviewed. We know this is difficult, and we will do our best to make this transition as smooth and sensitive as possible.
